Please note: Your registration with Team Coletta does not include your registration with the 2011 First Midwest Bank Southwest Half Marathon or 2011 Bank of America Chicago Marathon. You still need to register for the half marathon at www.palosbankhalfmarathon.com, or the marathon at www.chicagomarathon.com.

Waiver Release: I hereby certify the following: (1) I am physically fit and have received medical clearance to participate in the Bank of America Chicago Marathon or Palos Bank Southwest Half Marathon. (2) In consideration for my application to participate in the Bank of America Chicago Marathon, or Palos Bank Southwest Half Marathon, being accepted, I, on behalf of myself, my heirs and assigns, and my estate, hereby waive and forever discharge the sponsors, organizers, affiliates, as well as their agents and employees from any and all claims that may occur as the result of my participation. (3) I hereby grant the Bank of America Chicago Marathon, and Palos Bank Southwest Half Marathon, specific permission to reproduce, publish and circulate, copyright or otherwise use of any and all photographs and/or videotape of me and/or my family, taken at the Bank of America Chicago Marathon, or Palos Bank Southwest Half Marathon, for use by the St. Coletta's of Illinois Foundation.
